Job Summary
To provide high-quality, cost-effective healthcare outcomes, the full-time Mississippi Licensed Care Manager will develop, assess, and facilitate complex care management activities for members with physical needs while working remotely.
Key responsibilities
- Evaluates member needs and barriers to care, recommending personalized care plans for optimal outcomes
- Collaborates with healthcare providers to create ongoing service plans and address unmet member needs
- Monitors member progress and adjusts care plans as necessary, ensuring effective communication with members and caregivers
Required qualifications
- Degree from an accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN license in Mississippi or Compact State Licensure

- Knowledge of social determinants of health and community resources
