Mississippi Licensed Care Manager (RN)
Job Summary
To support complex care management activities, the full-time Mississippi Licensed Care Manager (RN) will develop personalized care plans, assess member needs, and facilitate access to healthcare resources while working remotely from Monday to Friday, 8:00 AM to 5:00 PM CST.
Key responsibilities:
- Evaluate member needs and barriers to care, recommending plans for optimal outcomes
- Develop ongoing care plans and collaborate with providers to address unmet needs
- Monitor member status and update care plans as necessary to meet goals
Required qualifications:
- Current, unrestricted Registered Nurse (RN) license in Mississippi
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in case management or discharge planning
