Job Summary
Providing personalized care and support to pregnant members, the full-time Missouri Licensed Care Manager (RN) will develop care plans, assess member needs, and facilitate access to appropriate healthcare resources while working remotely from anywhere within Missouri.
Key responsibilities
- Evaluates member needs and barriers to care, recommending plans for optimal outcomes
- Develops and collaborates on ongoing service plans with providers to address unmet needs
- Coordinates care management interventions and monitors member progress towards care goals
Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN - Registered Nurse State Licensure or Compact State Licensure
