Job Summary
Providing high-quality, cost-effective healthcare outcomes, the full-time Missouri Licensed Care Manager (RN) will develop and facilitate complex care management activities for pregnant members, working remotely within the state of Missouri, with a schedule of Monday to Friday, 8am to 5pm.
Key responsibilities
- Evaluates member needs and barriers to care, recommending personalized care plans for optimal outcomes
- Develops ongoing care plans and collaborates with providers to address members' unmet needs
- Coordinates care between members, families, and providers, ensuring timely access to necessary services
Required qualifications
- Degree from an accredited school of nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN licensure in Missouri or compact state licensure
