Missouri Licensed Care Manager RN
Job Summary
To support pregnant members, the full-time Missouri Licensed Care Manager RN will develop and facilitate complex care management activities, including personalized care plans and education, while working remotely within the state of Missouri, Monday through Friday from 8am to 5pm.
Key responsibilities:
- Evaluates member needs and barriers to care, recommending plans for optimal outcomes
- Develops and collaborates on ongoing care plans with providers to address unmet needs
- Coordinates care between members, families, and providers, ensuring person-centered services
Required qualifications:
- Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN - Registered Nurse state licensure or Compact State Licensure
