Job Summary
To support health needs in the Kansas City metropolitan area, the full-time Missouri Licensed Clinical Social Worker will utilize a collaborative process for assessment, planning, and advocacy while developing individualized care plans and monitoring their effectiveness.
Key responsibilities
- Completes member assessments and implements culturally sensitive care plans based on collected health information
- Monitors and adjusts care plans by gathering information from relevant sources to improve health outcomes

Required qualifications
- Bachelor's degree in social work/social services
- 3-5 years of experience in a health care setting
- Current Missouri or Kansas social work licenses; must obtain secondary license within 30 days of start date
