Job Summary
To support a growing healthcare team, the part-time Missouri Licensed Nurse Navigator will coordinate patient care, provide education, and collaborate with interdisciplinary teams remotely to ensure high-quality outcomes and seamless patient experiences.
Key responsibilities
- Coordinate care for assigned inpatient and outpatient populations, guiding patients through their treatment journey
- Provide patient and family education, including pre-operative preparation and follow-up care
- Support discharge planning and coordinate post-acute services while participating in quality improvement initiatives
Required qualifications
- Graduate of an accredited School of Nursing
- Current RN license in the state of practice
- Minimum of 2-3 years of nursing experience
- BLS certification
