Job Summary
To support patient care coordination, the full-time Missouri Licensed Nurse Navigator will oversee the delivery of care for inpatient and outpatient populations, ensuring timely and high-quality healthcare services while working remotely.
Key Responsibilities
- Coordinate and oversee patient care delivery, including assessments and individualized care plans
- Serve as a clinical resource and mentor for nursing staff, promoting professional accountability and regulatory compliance

Required Qualifications
- Graduate of an accredited School of Nursing
