Job Summary
To support pregnant members, the full-time Missouri Licensed RN Care Manager will develop and facilitate personalized care plans, assess member needs, and provide education and resources, all while working remotely within the state of Missouri.
Key responsibilities:
- Evaluate member needs and barriers to care, recommending plans for optimal outcomes
- Develop ongoing care plans and collaborate with providers to address unmet needs
- Provide education to members and families on healthcare options and resources
Required qualifications:
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN - Registered Nurse state licensure or compact state licensure
