Missouri Telephonic Care Navigator

Job Summary

Providing telephonic outreach to members, the full-time Missouri Telephonic Care Navigator will educate clients on care gaps, assist with scheduling appointments, and connect them to community resources while working remotely during standard business hours.

Key responsibilities:

• Completes telephonic outreach to DSNP/CSNP members using various outreach methods
• Educates members on care gaps and assists with scheduling provider appointments
• Links members to community resources and ensures access to primary care providers

Required qualifications:

• High School Diploma / GED or equivalent work experience
• 1+ years of call center and/or telephonic customer service experience
• 1+ years of professional experience in an office setting using telephone and computer
• 1+ years of healthcare, insurance, or social work/community outreach experience
• Ability to work full-time, Monday - Friday, with flexibility for 8-hour shifts
