Job Summary

To deliver comprehensive virtual care, the part-time contract Multi-State Licensed Telehealth NP will conduct new patient evaluations, manage chronic conditions, and perform Annual Wellness Visits remotely while collaborating with a virtual care team.

Key Responsibilities

• Conduct Annual Wellness Visits (AWV) and E/M virtual visits
• Perform new patient evaluations and follow-up consultations
• Manage chronic conditions and support remote patient monitoring

Required Qualifications

• Active, unrestricted NP license in three or more states, including MS, LA, IL, GA, or SC
• Proven clinical experience in preventative and/or primary care, with telehealth experience preferred
• Understanding of CMS credentialing processes
• Strong digital fluency and ability to navigate multiple systems
• Excellent EHR documentation habits
