Multi-State Licensed Telehealth Nurse Practitioner
Job Summary
To deliver comprehensive virtual care, the part-time contract Multi-State Licensed Telehealth Nurse Practitioner will conduct patient evaluations, manage chronic conditions, and perform Annual Wellness Visits remotely, requiring a minimum commitment of 16 hours per week.
Key Responsibilities
- Conduct Annual Wellness Visits (AWV) and E/M virtual visits
- Perform new patient evaluations and follow-up consultations
- Manage chronic conditions and support remote patient monitoring
Required Qualifications
- Active, unrestricted NP license in three or more states (MS, LA, IL, GA, SC)
- Proven clinical experience providing preventative and/or primary care
- Previous telehealth experience is strongly preferred
- Understanding of CMS credentialing processes
- Strong digital fluency and excellent EHR documentation skills
