Multistate Licensed Post-Acute Care Manager
Job Summary
To support patient transitions to post-acute facilities, the full-time Multistate Licensed Post-Acute Care Manager will manage a caseload, coordinate care with interdisciplinary teams, and ensure appropriate utilization of services while working remotely.
Key responsibilities
- Runs daily reports for transitions of care and reviews goals with care teams
- Communicates weekly with skilled nursing facilities to monitor patient care progression
- Conducts comprehensive assessments and coordinates referrals for transitions of care
Required qualifications
- Associates of Science in Nursing (ADN) required; Bachelor's of Science in Nursing (BSN) preferred
- Current RN License in the state of practice or covered by compact required
- 2 years of nursing experience required; 1 year in skilled nursing settings preferred
- Certification for care management preferred
