Multistate Licensed RN Case Manager

Job Summary

To support optimal transitions of care, the remote Multistate Licensed RN Case Manager will monitor real-time 'Admit, Discharge, Transfer' notifications, collaborate with ED and outpatient care teams, and manage patient outreach and discharge coordination.

Key responsibilities:

• Independently collaborates with ED and outpatient care teams to create individualized transition plans for members
• Acts as a clinical liaison with hospital staff to perform transition of care and care coordination for ED discharges
• Conducts outreach following ED discharge to confirm access and completion of discharge plans

Required qualifications:

• Current, unrestricted Multistate RN license
• 4+ years of diverse clinical experience in caring for acutely ill patients
• 1+ years of experience working in the Emergency Department
• Knowledge of utilization management and discharge planning
• Ability to recognize early signs of clinical deterioration and identify patients at risk for ED return visits
