Job Summary
Leading a remote team of Therapists and Care Extenders, the full-time salaried Nevada Licensed Clinical Program Manager will implement personalized case management for members, focusing on health and wellness outcomes while empowering the team to utilize local resources effectively.
Key responsibilities
- Manage and support the care team in developing and executing service plans tailored to individual member needs
- Monitor member progress and coordinate community resource guides to enhance behavioral health and violence prevention services
- Oversee team performance, ensuring adherence to Key Performance Indicators and conducting training and performance reviews
Required qualifications
- Master's degree in behavioral health, social work, or health care management from an accredited university
- Independent clinical behavioral health license in the state of Nevada (e.g., LCSW, CPC, LMFT, or Licensed Psychologist)
- Experience in leading and implementing programs with demonstrated outcomes
- Familiarity with health coaching or medical social work preferred
- Experience managing interdisciplinary care teams is preferred
