To support a growing healthcare team, the full-time Nevada Licensed Nurse Care Manager will manage patients with complex chronic conditions remotely, collaborating with healthcare providers to develop and implement comprehensive care plans aimed at optimizing patient outcomes and reducing unnecessary healthcare utilization.
Key responsibilities
- Quickly build empathetic relationships with patients and families to assess their individual needs
- Develop and implement care plans in coordination with physicians and interdisciplinary teams
- Monitor patient care and outcomes, making adjustments to care plans as necessary
Required qualifications
- Active Registered Nurse license in Nevada
- Experience engaging patients virtually using various technologies
- Bachelor's of Science in Nursing preferred
- 3+ years of clinical practice in a healthcare setting preferred
- Prior experience with electronic health records or health registries required
