Job Summary
Supporting the New Hampshire Health Families Medicaid population, the full-time New Hampshire Licensed Care Manager will develop and facilitate complex care management activities, focusing on both physical and behavioral health needs to ensure high-quality healthcare outcomes.
Key responsibilities
- Evaluates member needs and barriers to care, recommending personalized care plans for optimal outcomes
- Coordinates with providers and community resources to address unmet needs and ensure timely access to services
- Monitors member progress towards care plan goals and revises plans as necessary in collaboration with members and caregivers
Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN - Registered Nurse licensure in New Hampshire or compact state licensure
- Experience in both physical and behavioral health care management
- Ability to collect and document member information in compliance with regulatory standards
