New Jersey Licensed Care Manager

Job Summary

To support specialty populations, the full-time New Jersey Licensed Care Manager will perform care coordination duties, assess clinical needs, and implement interventions to ensure efficient utilization of benefits while working remotely.

Key responsibilities

• Assess member's clinical needs against established guidelines for specialty populations
• Implement high-quality, cost-effective care based on customized population models and clinical practice guidelines
• Partner with members, families, and healthcare teams to coordinate resource utilization and advocate for member needs

Required qualifications

• New Jersey RN license or a Bachelor's degree in social work, health, or behavioral science
• Graduate level degree in Social Work or Health and Behavioral Science preferred
• Minimum of three (3) years' experience serving members with special needs
• Strong knowledge of case management standards and managed care principles
• Valid New Jersey State License and Insurance for certain positions
