New Jersey Licensed Care Manager
Job Summary
To provide high-quality, cost-effective healthcare outcomes, the full-time New Jersey Licensed Care Manager (RN) will develop and facilitate complex care management activities for Medicaid and dual-eligible Medicare/Medicaid members, working remotely with occasional statewide travel.
Key responsibilities:
- Evaluate member needs and barriers to care, recommending personalized care plans for optimal outcomes
- Develop and collaborate on ongoing care/service plans, coordinating with providers and community resources
- Monitor member status and progress towards care goals, revising plans as necessary to meet individual needs
Required qualifications:
- Degree from an accredited school of nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN - Registered Nurse state licensure or compact state licensure
