New Jersey Licensed Care Manager

Job Summary

Providing support and advocacy for individuals receiving New Jersey Waiver Services, the remote New Jersey Licensed Care Manager will ensure high-quality service delivery, maintain compliance, and support both clients and direct care staff.

Key Responsibilities

• Review client service documentation to meet regulatory standards and maintain accurate records
• Conduct clinical meetings, develop individualized goals, and complete assessments and progress reports
• Resolve client grievances, oversee staffing patterns, and build community relationships to support resource access

Required Qualifications

• Master's degree with 1 year of experience, Bachelor's degree with 2 years, Associate's degree with 4 years, or High school diploma/GED with 5+ years of experience working with persons with disabilities
• Strong understanding of ISP implementation, goal writing, and Medicaid/third-party billing
• Ability to manage changing priorities and work in a fast-paced environment
• Excellent communication and interpersonal skills with clients, families, and community partners
