Job Summary
To support specialty populations, the full-time New Jersey Licensed Care Manager will perform care coordination duties, ensuring appropriate levels of care and planning transitions while working remotely.
Key responsibilities
- Assess members' clinical needs against established guidelines for specialty populations
- Implement high-quality, cost-effective care based on a customized population model
- Collaborate with members, families, and healthcare teams to coordinate resource utilization and advocate for members' needs
Required qualifications
- Licensed RN or Bachelor's degree in social work, health, or behavioral science
- Graduate-level degree in Social Work or Health and Behavioral Science preferred
- Minimum of three years' experience serving members with special needs
- Strong knowledge of case management standards and managed care principles
- Valid New Jersey State License and Insurance for certain positions
