Job Summary

Providing essential support to field-based Care Managers, the full-time New Jersey Licensed Clinical Care Associate will coordinate healthcare services, assist members with service needs, and ensure effective communication, all while working remotely.

Key responsibilities

• Assist with scheduling and coordinating service needs identified in members' Plans of Care
• Ensure timely follow-up with members and provide telephonic assistance for program-specific activities
• Document interactions accurately and comprehensively according to organizational policies

Required qualifications

• High School Diploma/GED required
• Preferred healthcare industry experience, with at least one year in home care, discharge planning, or case management
• Knowledge of managed care principles and care management standards
• Strong organizational skills and proficiency in Microsoft Office applications
• Ability to work with limited daily supervision
