Job Summary
Seeking a compassionate New Jersey Licensed Palliative LPN for a full-time remote position, this role focuses on providing ongoing clinical coordination, symptom management support, and education to patients and caregivers dealing with serious or life-limiting illnesses.
Key Responsibilities
- Manage scheduled telephone and virtual patient check-ins under the direction of the supervising RN/provider
- Conduct high-acuity patient chart reviews to assess hospice clinical appropriateness
- Monitor and document patient-reported symptoms, changes in condition, and care needs
Required Qualifications
- Current, unrestricted LPN license in New Jersey
- Graduate of an accredited practical nursing program
- Prior experience in palliative care, hospice, or related settings preferred
- Proficiency with electronic medical records and standard computer applications
- Strong interpersonal skills and ability to communicate with empathy and professionalism
