Job Summary
To support the Specialty Chronic Condition Plan, a full-time New Jersey Licensed Registered Nurse will perform comprehensive member assessments, coordinate individualized care plans, and advocate for members to ensure access to appropriate healthcare services, all while working remotely.
Key responsibilities
- Conduct comprehensive member assessments to determine appropriate levels of care and service needs
- Coordinate and assist with the implementation of individualized care plans across the continuum of care
- Advocate for members to ensure access to necessary healthcare services and resources
Required qualifications
- High School Diploma or GED required
- Minimum of two (2) years of clinical nursing experience
- Minimum of two (2) years of healthcare payer experience
- Active, unrestricted New Jersey Registered Nurse (RN) license or Compact RN license required
- Knowledge of case management, care management, and disease management principles
