New Jersey Licensed RN Home Care Coordinator

Job Summary

To support a growing team, the part-time New Jersey Licensed RN Home Care Coordinator will conduct in-home care assessments, coordinate care delivery, and collaborate with interdisciplinary teams while working a hybrid schedule that includes both remote and onsite responsibilities.

Key responsibilities:

• Coordinate incoming calls related to participants, physicians, and agency services regarding orders and referrals
• Communicate with participants and healthcare staff to manage schedules, test results, and care updates
• Participate in end-of-life care coordination and support activities

Required qualifications:

• Active, unrestricted Registered Nurse (RN) license in the state of New Jersey
• Bachelor's degree preferred
• Minimum of three (3) years of case management or nursing experience in a clinical or home setting with frail or elderly populations
• Nursing knowledge and training necessary to treat frail, elderly participants and manage complicated clinical conditions preferred
