New Jersey Licensed SNP Care Manager
Job Summary
To support the care of Dual Special Needs members, the full-time New Jersey Licensed SNP Care Manager will assess clinical and psychosocial needs, develop individualized care plans, and coordinate services while working remotely.
Key responsibilities
- Assess members' needs and develop member-centered care plans based on clinical guidelines
- Coordinate care with healthcare providers and advocate for members to ensure access to necessary services
- Monitor healthcare services and track member progress, adjusting care plans as needed
Required qualifications
- High School Diploma or GED required
- Minimum of three (3) years of clinical experience in home care, discharge planning, or case management
- Active, unrestricted New Jersey license as an RN, LPN, SW, or CSW
- Strong knowledge of case management standards and managed care concepts
- Experience working within the healthcare delivery system
