Job Summary
To enhance patient care coordination, the full-time salaried New Mexico Licensed Care Coordinator IV will facilitate interdisciplinary team collaboration, conduct comprehensive health assessments, and implement care strategies for clients with complex conditions in a remote setting.
Key responsibilities:
- Facilitates a team approach with the Interdisciplinary Care Plan team to ensure effective and cost-efficient delivery of quality care
- Conducts in-depth health risk and comprehensive needs assessments, addressing various member needs
- Provides care coordination for members with chronic or complex conditions, implementing and monitoring strategies to improve health outcomes
Required qualifications:
- Master's degree with 4 years of experience, or Bachelor's degree with 8 years, or Associate's degree with 9 years, or 12 years of relevant experience in lieu of formal education
- Proficiency in Microsoft Word, Excel, and Outlook
- Experience in analyzing trends using decision support systems
- Business management skills, including cost/benefit analysis and negotiation
- Knowledge of referral coordination to community and public/private resources
