Job Summary
To support a growing virtual healthcare team, a full-time New York Licensed Bilingual Nurse Practitioner will provide primary and urgent care via telehealth, develop clinical plans for complex adult patients, and collaborate with interdisciplinary teams to coordinate optimal patient care.
Key responsibilities
- Provide primary and urgent health care through telephone and tele video modalities to patients receiving home and community-based services
- Develop and implement clinical plans of care for adult patients facing chronic and complex conditions
- Educate patients and families about medication usage, illness progression, and medical adherence while maintaining communication with other healthcare providers
Required qualifications
- Master's degree in Nursing
- Active national Nurse Practitioner certification
- Valid and unrestricted Family or Adult Nurse Practitioner license in New York
- Valid and unrestricted RN license
- 2+ years of experience in managing complex care cases
