Job Summary
Managing an assigned member caseload, the full-time New York Licensed Care Coordinator will perform care management services, assist members in navigating their healthcare system, and support various teams to ensure excellent customer service remotely.
Key responsibilities
- Receive and document incoming calls from members regarding benefit questions, care coordination, and screening assessments
- Collaborate with the interdisciplinary care team to communicate member needs and monitor screenings for quality improvement
- Conduct outreach calls to confirm that services are in place for members requiring short-term care coordination
Required qualifications
- Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training or experience
- High school diploma or GED
- Experience in a fast-paced environment managing multiple priorities
- Effective verbal and written communication skills with good grammar and spelling
- Familiarity with medical terminology and healthcare environments is preferred
