Job Summary

To support patient engagement and health improvement, the remote New York Licensed Care Navigator will manage physiological markers, conduct wellness calls, and collaborate with care teams, working full-time within the EST time zone.

Key responsibilities

• Manage physiological markers such as blood pressure, weight, and blood glucose with clinical appropriateness
• Conduct monthly wellness calls with assigned patients and lead collaborative discussions to define health goals
• Provide preventive health education and coaching while ensuring patient engagement in remote patient monitoring programs

Required qualifications

• Unrestricted RN/LPN/LVN license in a compact state and New York
• 1-3 years of clinical experience in patient management or disease management
• Experience performing virtual visits and telephonic care management
• Ability to work independently with minimal direction and adapt to different provider practices
• Spanish speaking is a plus
