New York Licensed Case Management Nurse

Job Summary

To support comprehensive case management services, the full-time New York Licensed Case Management Nurse will conduct assessments, develop care plans, and coordinate care across the healthcare continuum while ensuring compliance with regulatory standards.

Key responsibilities

• Conduct comprehensive assessments to identify medical, behavioral health, and psychosocial needs
• Develop and implement individualized care plans in collaboration with members and interdisciplinary teams
• Coordinate care to promote quality outcomes and address barriers to treatment adherence

Required qualifications

• Current and unrestricted Registered Nurse (RN) license; New York State RN licensure preferred
• Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience considered
• Minimum two years of nursing experience in case management or related clinical settings
• Experience in behavioral health case management or psychiatric nursing preferred
• Knowledge of care management principles and healthcare delivery systems
