Job Summary
To enhance care delivery for patients in rural communities, the full-time salaried New York Licensed Clinical Coordinator will manage transitions of care, high-risk care management, and care coordination to reduce emergency department visits and hospitalizations while working in a hybrid environment.
Key responsibilities
- Coordinate transitions of care and conduct structured patient outreach after hospital discharges to evaluate follow-up needs
- Develop individualized care plans for high-risk patients with complex chronic conditions, utilizing motivational interviewing and health coaching
- Engage with multidisciplinary teams to address patient needs and ensure effective communication across care settings
Required qualifications
- Current, unencumbered Licensed Practical Nurse (LPN) license in the State of New York
- Proficiency with electronic health records and telehealth technology
- Strong clinical assessment and critical thinking skills in a high-volume environment
- Cultural humility and experience serving diverse and underserved communities preferred
- CPR/BLS certification current
