Job Summary

To enhance patient care in the HV region, the full-time New York Licensed Complex Care RN will provide telephonic triage, chronic disease coordination, and nursing support across multiple sites, ensuring timely and effective care delivery while working in a hybrid environment.

Key responsibilities

• Serve as a remote care-coordination resource, partnering with clinical teams to close care gaps and ensure continuity of care
• Conduct telephone medication reconciliation and structured post-discharge outreach to patients, addressing their needs and facilitating follow-ups
• Maintain accurate documentation of all coordination activities in the electronic medical record to ensure compliance and continuity of care

Required qualifications

• Current, unencumbered Registered Nurse (RN) license in the State of New York
• BSN preferred; ADN considered
• Experience in care coordination, chronic disease management, or telehealth/triage strongly preferred
• Proficiency with electronic health records and telehealth technology
• CPR/BLS certification current
