Job Summary

To enhance care coordination for patients with complex chronic conditions, the full-time New York Licensed Complex Care RN will provide telephonic triage, chronic disease management, and support to care teams across multiple sites, operating within a hybrid framework from Monticello, NY.

Key responsibilities

• Serve as a remote care-coordination resource, partnering with site providers and care teams to ensure continuity of care
• Conduct telephone medication reconciliation and structured post-discharge outreach to assigned patients
• Deliver telephonic patient education on medications and self-care measures while maintaining accurate documentation in the electronic medical record

Required qualifications

• Current, unencumbered Registered Nurse (RN) license in the State of New York
• BSN preferred; ADN considered
• Experience in care coordination, chronic disease management, or telehealth/triage strongly preferred
• Proficiency with electronic health records and telehealth technology
• CPR/BLS certification current
