Job Summary

To support patients enrolled in chronic care management and remote patient monitoring programs, a full-time Care Navigator will leverage their New York Licensed LPN credentials to educate and empower patients while maintaining accurate documentation and collaborating with healthcare teams remotely.

Key Responsibilities:

• Complete phone consultations with patients, providing support and education about their chronic conditions
• Educate patients on health conditions and assist them in setting and achieving goals aligned with their care plans
• Maintain accurate patient records and respond to remotely transmitted patient data according to established workflows

Required Qualifications:

• Current, valid, and in good standing New York Nursing License (LPN/LVN)
• 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management
• Proficiency in using electronic health records (EHR) and care management software
• Reliable internet connection and proficiency with virtual communication tools
• Empathy and a patient-centered approach to care
