Job Summary

To support a diverse patient population, the part-time New York Licensed Nurse Practitioner will provide high-quality, evidence-based primary care through asynchronous text-based chat and synchronous video, managing complex and chronic illnesses while collaborating with a clinical care team.

Key responsibilities

• Deliver best-in-class care to patients with complex and chronic medical conditions using innovative technology
• Provide compassionate and empathetic support to patients across all age groups and backgrounds
• Collaborate with team members to efficiently solve patient problems and optimize care outcomes

Required qualifications

• 5+ years of advanced clinical experience in complex care, chronic care management, or primary care
• Active Nurse Practitioner license in New York; multi-licensure in other states preferred
• Board certification as a Nurse Practitioner
• Experience or willingness to learn digital-first healthcare practices
• Proficiency with technology and applications for virtual patient care
