New York Licensed Nurse Practitioner Supervisor

Job Summary

To support a growing interdisciplinary team, the part-time New York Licensed Nurse Practitioner Supervisor will manage clinical care delivery, coordinate care with various providers, and participate in the development of clinical programs while working remotely.

Key responsibilities

• Represent the primary care provider in site-level meetings and manage high-risk participants
• Conduct comprehensive patient assessments via telehealth and provide evidence-based clinical care
• Collaborate with interdisciplinary teams to develop and implement population health initiatives

Required qualifications

• Graduated from an accredited Nurse Practitioner education program and licensed to practice in New York
• Minimum of three years of Nurse Practitioner experience, preferably in primary care or population health
• Experience with telehealth and clinical program development preferred
• DEA Certification and AANP or ANCC Board Certification required
• Strong clinical assessment skills and proficiency with Electronic Health Records (EHRs)
