New York Licensed Practical Nurse Coordinator

Job Summary

To ensure high-quality care services, the full-time New York Licensed Practical Nurse Coordinator will work remotely on weekends, managing patient care plans, coordinating with clinicians, and maintaining communication with patients to achieve healthcare goals.

Key responsibilities:

• Complete individualized patient care plans and perform care management using electronic medical records
• Maintain frequent contact with patients to provide support and manage compliance with care management programs
• Document all client communications accurately and keep the Team Supervisor informed of care plan issues

Required qualifications:

• Must be a Licensed Practical Nurse (LPN)
• Comfortable with extensive phone communication
• Exceptional customer service skills and a compassionate demeanor
• Ability to multitask and remain organized in a fast-paced environment
• Proficiency in using computers and Microsoft Office programs
