New York RN Care Manager

Job Summary

To support a growing care management team, the full-time New York RN Care Manager will assist enrollees in accessing necessary benefits, coordinate services according to care plans, and serve as the primary contact for members while working remotely.

Key responsibilities

• Assess, plan, and implement individualized care strategies for members
• Utilize the UAS-NY to manage Person Centered Service Plans effectively
• Advocate for members and communicate health-related information to ensure quality care coordination

Required qualifications

• Associate's degree
• Current, unrestricted RN license for the state of New York
• 4+ years of experience in a community health setting
• 2+ years of experience in long-term care, home health, hospice, public health, or assisted living
• 1+ year of relevant community case management experience
