New York RN Care Manager
Job Summary
To support a diverse patient population, the full-time RN Care Manager will manage care planning, coordination, and health assessments while working in a hybrid environment and residing within the New York Tri-State Area.
Key responsibilities
- Oversee care management processes, including care planning and coordination for patients
- Monitor the quality and effectiveness of services, tracking progress towards individual health goals
- Facilitate smooth transitions and discharge planning for patients from hospitals or nursing homes
Required qualifications
- Valid NYS RN License
- Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings
- Associate's degree or higher
- Bilingual proficiency in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole
- Ability to effectively utilize various software and tools relevant to care management
