NH Licensed Primary Care APP

Job Summary

Supporting primary care teams remotely, the full-time NH Licensed Primary Care APP will manage indirect patient care tasks, including responding to patient inquiries and coordinating care with on-site providers.

Key responsibilities

• Respond to patient calls and portal messages requiring provider input
• Review and refill medication requests while coordinating patient care with triage and care coordination nurses
• Communicate lab and diagnostic test results to patients and attend virtual meetings with on-site providers

Required qualifications

• Master's Degree in a relevant field
• 3-5 years of clinical experience in primary care
• Board-certified APRN or PA-C
• DEA registration
• New Hampshire License
