NH Licensed Primary Care Provider

Job Summary

To support primary care teams remotely, the full-time NH Licensed Primary Care Provider will manage indirect patient care tasks, including responding to patient inquiries and coordinating care with on-site staff.

Key responsibilities

• Respond to patient calls and portal messages requiring provider input
• Review and refill medication requests and communicate lab results to patients
• Coordinate patient care with triage and care coordination nurses

Required qualifications

• Master's Degree in a relevant field
• 3-5 years of clinical experience in primary care
• Board-certified APRN or PA-C
• Current DEA registration
• NH License to practice as a primary care provider
