Job Summary

Supporting primary care teams remotely, the full-time NH Licensed Remote Primary Care Provider will manage indirect patient care tasks, collaborate with on-site providers, and ensure comprehensive medical care for patients.

Key responsibilities

• Respond to patient calls and portal messages requiring provider input
• Review and refill medication requests while coordinating care with triage and care coordination nurses
• Communicate lab and diagnostic test results to patients and attend virtual meetings with on-site providers

Required qualifications

• Master's Degree in a relevant field
• 3-5 years of clinical experience in primary care, covering all ages
• Board certification as an APRN or PA-C
• DEA certification
• NH License; New England residency preferred
