North Carolina Licensed Care Manager
Job Summary
To provide high-quality, cost-effective healthcare outcomes, the full-time North Carolina Licensed Care Manager (RN) will develop and facilitate complex care management activities for approximately 73 members, working remotely with a flexible schedule after training.
Key responsibilities
- Evaluate member needs and barriers to care, recommending personalized care plans and interventions
- Collaborate with healthcare providers and community resources to address unmet needs and ensure timely access to services
- Monitor member progress towards care plan goals and provide ongoing education to members and their families
Required qualifications
- Active North Carolina RN license required
- Degree from an accredited School of Nursing or a Bachelor's degree in Nursing
- 2-4 years of related experience in care management or healthcare
- Ability to work remotely and travel approximately 10% for meetings and team gatherings
- Experience in developing care plans and collaborating with healthcare providers
