Job Summary
Managing care for members with complex, high-risk needs, the full-time North Carolina Licensed Care Manager will conduct assessments, develop care plans, and connect members with appropriate resources while working remotely.
Key responsibilities
- Evaluate member needs and barriers to care, recommending plans for optimal outcomes
- Develop and collaborate on ongoing care plans with providers to address unmet needs

Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- North Carolina RN state licensure or Compact State Licensure
- Experience in maternity care management and working with the pediatric population
