North Carolina Licensed Case Manager
Job Summary
To support a leading organization in the managed care and health insurance industry, the remote North Carolina Licensed Case Manager will conduct clinical assessments, develop care plans, and coordinate healthcare services to improve clinical outcomes.
Key responsibilities
- Conduct comprehensive clinical assessments and develop individualized care plans addressing medical, behavioral, and social needs
- Coordinate services across various healthcare settings and maintain ongoing communication with members and care teams
- Educate members on treatment options, available benefits, and community resources to support positive health outcomes
Required qualifications
- Active North Carolina RN license or Compact RN license in good standing
- Minimum of 3 years of clinical nursing and/or case management experience
- Prior telephonic case management experience within a health insurance carrier or managed care organization
- Experience supporting Medicare and/or Medicaid populations, with a preference for behavioral health or adolescent case management
- Strong care coordination and assessment skills with the ability to work independently in a remote setting
