North Carolina Licensed Case Manager

Job Summary

To enhance healthcare coordination, the remote North Carolina Licensed Case Manager will conduct clinical assessments, develop care plans, and facilitate communication among members, providers, and care teams.

Key responsibilities:

• Conduct comprehensive clinical assessments and create individualized care plans addressing medical, behavioral, and social needs
• Coordinate services across various healthcare settings and maintain ongoing communication with members and care teams
• Educate members on treatment options, community resources, and preventative health measures to support positive outcomes

Required qualifications:

• Active North Carolina RN license or Compact RN license in good standing
• Minimum of 3 years of clinical nursing and/or case management experience
• Prior telephonic case management experience within a health insurance carrier or managed care organization
• Experience supporting Medicare and/or Medicaid populations; behavioral health experience preferred
• Strong care coordination and documentation skills with the ability to work independently in a remote setting
