Job Summary
To support the Medicare Case Management team, a full-time North Carolina Licensed Case Manager will coordinate care and resources for selected member populations, performing comprehensive assessments and developing member-centric care plans while working remotely with occasional in-office visits.
Key responsibilities
- Coordinate care and resources as part of a multidisciplinary team to achieve optimal clinical outcomes
- Perform comprehensive assessments and develop tailored care plans based on members' needs and barriers
- Monitor and manage member progress, ensuring appropriate follow-up and documentation of care plans
Required qualifications
- RN with 3 years of clinical and/or case management experience required
- Valid and applicable clinical license (NC or compact multi-state licensure) to perform job duties
- Additional specialty certification (i.e. CCM, CDCES) may be required within 2 years of employment
