Job Summary
To support the Commercial Case Management team, the full-time North Carolina Licensed Case Manager will coordinate care and resources for selected member populations, utilizing clinical skills to develop member-centric plans and monitor ongoing care remotely.
Key responsibilities
- Coordinate care and resources for members as part of a multidisciplinary team to achieve optimal clinical outcomes
- Conduct comprehensive assessments and develop tailored plans addressing members' needs and barriers to care
- Monitor and manage member progress, providing education and referrals as necessary to facilitate positive health outcomes
Required qualifications
- Active RN license with 3 years of clinical and/or case management experience
- Valid North Carolina or compact multi-state clinical license
- Ability to obtain relevant specialty certification (i.e., CCM, CDCES) within 2 years of employment if required
- Experience in various clinical settings such as Medical-Surgical, ICU, or Emergency Department is preferred
- Strong understanding of healthcare resources and care planning processes
