North Carolina Licensed Case Manager RN

Job Summary

To support its Medicare Care Management team, a remote contract opportunity is available for a North Carolina Licensed Case Manager RN who will conduct telephonic case management for Medicare members, focusing on those with complex medical needs and facilitating care coordination across various providers.

Key Responsibilities

• Conduct comprehensive clinical assessments to evaluate members' healthcare needs and determine appropriate interventions
• Collaborate with members, families, and healthcare providers to develop individualized care plans
• Monitor members' progress towards established goals and adjust care plans based on clinical needs

Required Qualifications

• Active, valid Registered Nurse (RN) license in an approved state
• Minimum of 3 years of clinical and/or case management experience
• Strong clinical assessment and care coordination skills
• Excellent telephone communication skills and professional phone etiquette
• Ability to manage a caseload independently while prioritizing members according to clinical acuity
